Authorization & signature

Please read and sign the payment authorization agreement.

By signing this agreement, the account holder authorizes AAA Forklifts, Parts & Service to charge the payment method(s) provided through QuickBooks' secure payment portal for services performed on the customer's equipment.

A written or verbal acceptance of each estimate is required before any charges are made. The account holder will be charged the invoice amount once services are completed.

For credit or debit card charges, charges will appear on the cardholder's statement under "AAA Forklifts, Parts & Service".

For ACH bank account debits, charges will appear as an "ACH Debit" on the account holder's statement. In the case of an ACH transaction being rejected for Non-Sufficient Funds (NSF), AAA Forklifts may attempt to process the charge again within 30 days, and the account holder agrees to an additional $35 charge for each NSF attempt.

This authorization remains in effect until cancelled in writing. The account holder agrees to notify AAA Forklifts in writing of any changes in account information or termination of this authorization at least 15 days prior to the next billing date.

The account holder acknowledges that the origination of ACH transactions must comply with U.S. law, certifies being an authorized user of the payment method(s) provided, and agrees not to dispute scheduled transactions so long as they correspond to the terms indicated in this authorization.
