Please read before you proceed

How we bill for service

  • Service call: $150 within a 60-mile radius.
  • Labor: one-hour minimum. After the first hour, labor is billed in half-hour increments.
  • Travel time: billed in quarter-hour increments.
  • All hours worked: your card will be billed for all hours worked on the job site, unless otherwise stated. An estimate is our best estimate of the work. If a job takes longer than estimated, the additional hours worked are billed.

When your card is charged

  • When you accept an estimate, we charge the service call and the labor for a planned maintenance (PM) or diagnostic visit right away.
  • The rest of the estimate is authorized on your card at the same time. An authorization is a temporary hold, not a charge.
  • The balance, including any additional hours worked, is charged when the job is complete, and you'll receive an invoice for the work.

These billing terms are part of our Terms of Service.

A temporary $1.00 authorization will verify your card. When you submit, we place a one-time $1.00 hold on your card to confirm it's valid, then void it immediately — this is not a charge. Your bank may briefly show it as "pending" before it drops off, usually within 1-7 days.

Authorization & signature

Please read and sign the payment authorization agreement.

By signing this agreement, the account holder authorizes AAA Forklifts, Parts & Service to charge the payment method(s) provided through QuickBooks' secure payment portal for services performed on the customer's equipment.

A written or verbal acceptance of each estimate is required before any charges are made. When an estimate is accepted, the account holder authorizes AAA Forklifts to charge the service call and the labor for a planned maintenance or diagnostic visit at that time, to place an authorization for the rest of the estimate, and to charge the balance, including all hours worked on the job site, when the job is complete, as described under "Please read before you proceed" at the top of this form.

For credit or debit card charges, charges will appear on the cardholder's statement under "AAA Forklifts, Parts & Service".

For ACH bank account debits, charges will appear as an "ACH Debit" on the account holder's statement. In the case of an ACH transaction being rejected for Non-Sufficient Funds (NSF), AAA Forklifts may attempt to process the charge again within 30 days, and the account holder agrees to an additional $35 charge for each NSF attempt.

This authorization remains in effect until cancelled in writing. The account holder agrees to notify AAA Forklifts in writing of any changes in account information or termination of this authorization at least 15 days prior to the next billing date.

The account holder acknowledges that the origination of ACH transactions must comply with U.S. law, certifies being an authorized user of the payment method(s) provided, and agrees not to dispute scheduled transactions so long as they correspond to the terms indicated in this authorization.
